Eligibility Criteria
Due to the nature of the Supportive Living Program, where staff are not present 24/7, individuals must meet specific criteria to qualify for this living arrangement. A team of professionals—including the case manager, legal guardians (if applicable), medical professionals, and others involved in the individual’s care—will collaboratively assess whether the individual is a good fit for this level of independence. The decision is made to ensure the individual’s safety and well-being while supporting their desire for greater autonomy.
Personalized Care and Support
While our residents enjoy significant autonomy, we understand that support must always be accessible. Staff are available 24/7 to ensure that assistance is always within reach. Our staff conduct daily check-ins to monitor well-being, address any concerns, and offer guidance as needed. We assist with medication management, ensuring that all medication regimens are followed according to the individual's plan. We also follow each individual's ISP (Individualized Service Plan), tailoring our support to help residents grow and gain new skills, encouraging continuous personal development.
A Comfortable Home Environment
Residents will enjoy their own private bedroom, offering personal space to relax and unwind. While Same Life Services provides fully furnished living spaces, we encourage residents to personalize their rooms and decorate according to their own preferences, fostering a sense of ownership and comfort in their new home. The neighborhood surrounding the home includes great amenities, such as a basketball court and pool, adding to the enjoyment and engagement of the community.
Holistic Care for an Enriched Life
Our program goes beyond just meeting the basic needs of our residents. We provide transportation to a variety of destinations, including work, medical appointments, shopping trips, and community outings. We assist with daily living skills, such as meal preparation, budgeting, personal hygiene, and household tasks.
